Employee is killed falling into crusher

On August 9, 2001, an employee was operating the conveyor of a crushing and pulverizing machine manufactured by William Patent Corp. The employee climbed up the conveyor at a 45 degree incline to remove an object from the conveyor. He was not aware of his proximity to the interior of the crusher and fell inside it. The employee died from the fall.
