Investigation Summary
At approximately 4:30 a.m. on February 17, 2021, Employee #1 was cleaning under the running L3 Rademacher conveyor at a food processing facility. While wiping flour from a roller with a paper towel, the employee's right-hand glove was pulled into the machine, causing the forearm to become caught between the roller and a bar under the conveyor. As a result, the employee sustained a broken wrist and four breaks in the forearm. The employee was admitted and treated at the hospital for the injuries.
