Line 3 Rademacher - On or about January 3, 2021, an authorized employee performed preventive maintenance on the line 3 Rademacher machine (order no. 7700283). The employer failed to provide a periodic inspection that included a review between the inspector and each authorized employee and the employee's responsibility under the energy control procedure. b.) Line 3 Rademacher - On or about February 17, 2021, an authorized employee performed the service task of dismantling the laminator guard on the line 3 Rademacher machine. The employer failed to provide a periodic inspection that included a review between the inspector and each authorized employee and the employee's responsibility under the energy control procedure.
