Investigation Summary
On November 20, 2018, Employee #1 was on the middle section of a water tower 110 feet high. Employee #1 was wearing a lanyard attached to a painters ring. Employee #1 needed to move cables from a suspended scaffold, but before he reached to move the cables he fell 60 feet, injuring his back. Employee #1 was hospitalized and suffered paralysis from the waist down. The lanyard was a double lanyard fall restraint, and was still attached to the painter's ring. On December 7, 2018, Employee #2 was descending the ladder inside the water tower, and slipped about 10 feet before reaching the middle platform. Employee #2 suffered a broken right foot, and a laceration to his forehead and was hospitalized. Employee #2 was not using a lifeline, or ladder safety device.
