Employee killed when caught in coil handling equipment

The production control manager asked two employees from the shipping and receiving department to help transfer coils of slit metal weighing 136 lb from a turnstile to a downender being used in the automatic mode. When one employee was called away, Employee #1 and the manager continued the coil transfer. After they transferred a coil to the downender and activated the auto cycle down button, they removed an inner cork from the coil, and the production control manager stored it. When the manager heard the employee calling for help, he saw Employee #1 caught between the downender horn and the turnstile. The manager tried to reverse the operation of the horn but was unable to do so. He then pressed the emergency stop button. The hydraulic line had to be cut to free the employee. Employee #1 died.
