EMPLOYEE INHALES GASOLINE VAPORS AND DROWNS IN GASOLINE

EMPLOYEE #1, AN AVIATION SERVICE TECHNICIAN WAS REFILLING A TANK TRUCK BY HIMSELF. HE PLACED THE DEADMAN SWITCH BEHIND A CONDUIT TO BYPASS IT AND STOOD ON THE FENDER TO LOOK IN THE OPEN TANK HATCH. EMPLOYEE #1 APPARENTLY INHALED OCTANE VAPOR, LOST CONSCIOUSNESS, AND DROWNED IN THE GASOLINE BEING PUMPED OUT OF THE OPEN HATCH. EMPLOYEE #1 WAS FOUND BY CO-WORKERS AFTER APPROXIMATELY 30 MINUTES, DURING WHICH TIME SEVERAL HUNDRED GALLONS OF GASOLINE WERE SPILLED BY THE BYPASSED FUEL PUMP. NO FIRE OR EXPLOSION OCCURRED, DUE TO RAPID EMERGENCY RESPONSE. OBSERVATION OF STANDARD OPERATING PROCEDURES, INCLUDING THE USE OF THE HAND-HELD SHUTOFF SWITCH AND NOT STANDING OVER THE HATCH, WOULD HAVE PREVENTED THE EXPOSURE AND DROWNING. THE EMPLOYER HAD MADE NO EFFORT TO ENSURE THAT EMPLOYEES WERE COMPLYING WITH THESE PROCEDURES.
