Investigation Summary
At approximately 7:25 p.m. on December 15, 2019, an employee was operating the 50-inch line. The employee reached into the Beck shear on the line to remove a jam without stopping or locking out the equipment. The employee amputated four fingers on his left hand and was hospitalized to treat these injuries.
