Employee Amputates Finger Tips While Reloading the Chain of

At 9:00 a.m. on July 17, 2012, an employee was assisting a maintenance worker who was putting a chain back on its track for a lift truck carriage. The employee was holding onto the chain and the tips of his middle finger and ring finger on his right hand got caught between the mast and chain sprocket. Both finger tips were amputated.
