Investigation Summary
At 12:15 p.m. on June 10, 2014, Employee #1 had completed his lunch, which lasted less than the normal 30 minutes. Due to weather conditions, Employee #1 resumed diving. At about five minutes into the activity, a co-worker on the surface did not get a reply from his air supply line signal. Co-worker #1 requested the air supply line, Co-worker #2 disconnected the air supply line to add the Y and add another air line for Co-worker #1. Co-worker #2 stated that it took him 3 seconds to unhook and hook the Y and the air supply line up to Co-worker #1 . Co-worker #1 dove in and began searching for Employee #1. He was found with his weight belt entangled with an unknown source. Employee #1 was wearing a diving mask, wet suit, fins and weight belt. Co-worker #1 released the belt and Employee #1's body began to surface. The report from the medical examiner revealed severe coronary atherosclerosis while performing diving operations.
