Investigation Summary
At 1:30 p.m. on May 21, 2013, an employee was cleaning the waste conveyor room, which included cleaning beneath the conveyor's powered roller (drum) at the tail pulley section. The employee was manually cleaning wood cuttings (excelsior) from around the unguarded tail pulley section of a waste conveyor while the conveyor was running. The employee's right hand and arm were pulled into an in-going nip-point, where the conveyor belt ran into a powered drive roller (drum). The employee was asphyxiated.
