Employee crushed by falling glass pack

Employee #1 was working as a glass processor on the cold end of the main line. A jumbo glass pack, comprising nine liters of glass, each 128 in. by 0.5 in. and weighing about 1,134 lbs, was positioned on a tilt table. The employee was required to pick the pack off the table with a glass grabber (manufactured in house), a device weighing approximately 1,140 pounds. The glass grabber was suspended by a 5-ton, pendant-controlled, monorail hoist, manufactured by the American Crane Company. The employee was to use the glass grabber to place the pack onto a large wagon. He performed this task without assistance. He operated the grabber and crane controls to release the glass pack onto the wagon. He then began trolleying the hoist away. The glass pack toppled and struck him. He sustained severe lacerations and crushing-type injuries and died about an hour after the accident. The monorail hoist and load line had been overloaded. The top feet of the glass grabber may have hung up on broken strap brackets on the top frame of the wagon.
