EMPLOYEE #1 WAS DRIVING A FORKLIFT TRUCK FROM THE COMPANY YARD TO AN OIL WELL DRILLING LOCATION. HE LOST CONTROL OF THE FORKLIFT AND RAN OFF THE ROAD. THE FORKLIFT TURNED OVER ON ITS SIDE. THE DRIVER WAS DECAPITATED.
