Event: 07/02/2012

An employee using a forklift was found dead on the floor.

At 4:25 p.m. on July 2, 2012 at approximately 4:25 pm, Employee #1 was tasked to go to the point of sales warehouse and pick up materials to load on a truck. Employee #1 was operating a Yale Propane 2.5K Forklift (Model numer GLC040SVXNUSE088; serial number C809V02018D). Employee #1 was found lying on the floor. The forklift was still in gear and running. The forklift was found approximately 48 feet away from the body. Employee #1 was transported to the Hospital and pronounced dead on arrival. Employee #1 was killed from blunt force trauma to the head and cervical fractures.
