Employee struck and killed by a saw

At 8:10 a.m. on July 30, 1994, Employee #1 and a coworker were dislodging a log end. Another coworker, who had intended to shut off the electrical power to an approximately 52 in. cross cut saw, inadvertently activated the saw cycle control. The saw cycled and struck and killed Employee #1.
