Employee killed when run over by trolley wheel

Between 1:10 and 1:15 p.m. on March 19, 1986, Employee #1 arrived to adjust a level limit switch on the north transfer trolley as he had been assigned. The trolley operator told him that because they were running short on truck bodies, he had to work during his break and there was not time for him to adjust the switch. Employee #1 suggested that he come back at 2:30 p.m. and the operator said that would be great. Employee #1 departed and the operator continued to operate the trolley. At approximately 1:30 p.m., the operator stepped off the trolley to talk to a coworker and left the trolley cab momentarily. Employee #1 returned to the trolley and, seeing the operator out of the trolley cab, apparently thought that he could adjust the switch while the operator was gone. The operator returned to the trolley and, not knowing that Employee #1 was in front, started the trolley forward. The trolley ran over Employee #1 and killed him.
