EMPLOYEE FELL WITH LADDER; FRACTURED ARM AND TOE

AT APPROXIMATELY 9:30 AM ON JANUARY 10, 1986, EMPLOYEE #1 WAS USING AN EXTENSION LADDER TO CLIMB TO THE TOP OF A MANUALLY CONTROLLED OVERHEAD PULL DOOR TO REPAIR A SPRING. THE SPRING WAS LOCATED ABOUT 12 FEET ABOVE FLOOR LEVEL. THE LADDER WAS SET AT A VERY SHARP (UNSTABLE) ANGLE. WITHOUT ANY WARNING THE LADDER FEET BEGAN TO SLIDE OUTWARD, CAUSING THE LADDER TO FALL. EMPLOYEE #1 WRAPPED HIS RIGHT ARM AROUND THE LADDER WHILE HE RODE IT TO THE FLOOR. HE LANDED ON HIS RIGHT SIDE, FRACTURING HIS RIGHT ARM AND THE BIG TOE ON HIS RIGHT FOOT.
