HAND CRUSHED BY MALFUNCTIONING WORK PLATFORM

EMPLOYEE #1 WAS WORKING FROM A WORK PLATFORM ON A CINCINNATI GILBERT HORIZONTAL BORING MILL AT THE TIME OF THE ACCIDENT. ATTACHED TO THE PLATFORM WERE SUPPORT POSTS. THESE POSTS ARE FITTED THROUGH POST GUIDES WHICH ARE ATTACHED TO THE HEAD OF THE MACHINE. CAPS WERE PLACED ON THE ENDS OF THE POSTS. WHEN THE HEAD OF THE MACHINE IS RAISED, THE POST GUIDES COME IN CONTACT WITH THE CAPS AND RAISE THE PLATFORM. WHEN THE HEAD IS LOWERED, THE PLATFORM WILL LOWER. AT THE TIME OF THE ACCIDENT, AS THE HEAD WAS BEING RAISED, THE POST GUIDES BOUND UP ON THE POSTS ABOUT 10 FEET BELOW THE CAP, CAUSING THE PLATFORM TO RAISE. WHILE THE PLATFORM WAS IN THE RAISED POSITION, THE EMPLOYEE WALKED TO THE EDGE OF THE PLATFORM AND TOOK HOLD OF THE SUPPORT POST, PLACING HIS HAND BETWEEN THE POST GUIDE AND THE CAP. HE THEN LEANED OUT FROM THE PLATFORM TO INSPECT HIS WORK. THE PLATFORM THEN FELL TO IT'S STABLE POSITION, CRUSHING EMPLOYEE #1'S HAND BETWEEN THE CAP AND THE POST GUIDE.
