EMPLOYEE #1 WAS ADJUSTING THE LIMIT SWITCH WITH THE MACHINE CONTROLS NOT SET IN THE SAFE GUARDING MODE. THE MACHINE WAS ACTIVATED, CAUSING IT TO CYCLE. THE EMPLOYEE WAS CRUSHED BETWEEN THE MACHINE AND ITS FRAME.
