Investigation Summary
On December 23, 2013, Employee #1, a truck driver, was delivering medium-density fibreboard (MDF) plywood boards to a facility, where audio products were manufactured for household and commercial use. Each of the MDF boards measured 10 ft by 5 ft by 0.75 in. and were tied in bundles of 18 pieces each. They were double stacked on the flatbed truck. A coworker, who was operating a forklift, picked the top bundle with the forks and moved the forklift back away from the truck. As he lowered the forks, Employee #1 moved in between the forklift and the delivery truck to pick up spacer pieces, used in between the bundles of MDF boards. The coworker then pressed the wrong control lever on the forklift, which tilted load forward. The plywood bundled load fell, striking Employee #1. He sustained multiple fractures to various parts of his body. Employee #1 was transported to a medical center, where he received treatment and was then hospitalized for 24 days.
