Employee Is Struck by Forklift and Loses Lower Leg

On July 12, 2013, Employee #1 was standing on the forks of forklift that was being driven by a coworker. Employee #1 fell off the forklift and his lower right leg was run over by the fork truck mast. His lower right leg sustained traumatic injuries. Employee #1 was transported to a local hospital, where his lower leg was removed below the knee.
