Investigation Summary
At approximately 10:45 p.m. on February 20, 2014, Employee #1, a driver, was maneuvering his moving truck to keep it from crashing. As he navigated the vehicle, he slipped and fell from its cab and struck the ground. Employee #1 sustained serious injuries to his left clavicle and shoulder. He was transported by ambulance to a medical center, where he underwent surgery on shoulder and was then hospitalized.
