Employee Loses Part of Thumb in Press

An incident occurred as Employee #1 was operating a mechanical power press. Employee #1 was punching holes in an actuator bracket used in the automotive industry. Employee #1, asked his coworker how to switch from the hand controls to the foot control. The coworker showed Employee #1 where the switch was located. Employee #1 switched from the hand controls to the foot control and proceeded to operate the machine. Employee #1's thumb was between the die when the press was activated causing an amputation of the left thumb. Employee #1 was taken to a local area hospital where he was treated and released, they were unable to reattach the thumb.
