Employee Falls from Roof and Is Killed

On April 27, 2013, Employee #1 and a crew were building an 800 square foot addition on a single family home. They were applying roofing paper to the plywood sheathing on a roof under construction. The roof had a 2:12 (9.46 degree) slope. To access the addition, they traveled from the original roof. As Employee #1 was transferring from the original roof to the roof under construction. He lost his balance and fell 16.71 ft. Employee #1 struck the ground and sustained traumatic injuries that killed him.
