Investigation Summary
At approximately 9:00 a.m. on April 15, 2013, an accident occurred as Employee #1 was mixing a batch of industrial adhesive at a client work site in. Employee #1 became entangled with the rotating exposed vertical shaft of a mixer. Employee #1 was taken to a local hospital where he was pronounced dead.
