Investigation Summary
At approximately 12:00 a.m. on February 25, 2014, Employee #1, a sanitation worker, and a crew of three coworkers were cleaning handsorting tables in a pistachio processing facility. Employee #1 was cleaning the bottom section of one of the table with the conveyor belt running. While cleaning this bottom section, she lost her balance and grabbed the pulley for support. Her arm was caught and fractured in the belt support roller assembly. Employee #1 was transported to a medical center, where she received treatment and was then hospitalized.
