Employee Suffers Heat Illness

On August 14, 2012, Employee #1, a truck driver, was suffering from heat illness. His wife reported the condition on that day. Employee #1 was transported to the emergency room of a medical clinic, where he was treated and released. Personnel within the emergency room stated that Employee #1 did not inform them of his heat illness. On the day of incident, he had worked 6.75 hr with only 1.75 hr of actual drive time. There was no "log-on" of the Employee #1 ever entering the yard on the day of the injury. A worker comp insurance carrier submitted a claim notice from Employee #1 on December 17, 2012, but the claim was denied.
