Investigation Summary
On November 14, 2012, Employee #1 of Hi Side Fabrication, Inc., was working on setting a die on a punch press but was not wearing safety glasses. His coworker activated the punch press and a piece of the die broke off and stuck Employee #1, injuring the left eye. Employee #1 was taken to the Emanuel Hospital in Turlock and then airlifted to UC Davis Med center where he lost the eye.
