Investigation Summary
On September 03, 2013, Employee #1 was installing a main pipe in a 15 ft deep trench on the street. The pipe lifted catching the employee's feet between the shoring box and the pipe. Coworkers came to his assistance and lifted the pipe off the employee's foot. Employee #1 was taken to Antelope Valley Hospital, where he was treated for fractures to both feet. The employee was hospitalized for two days.
