Investigation Summary
At approximately 10:45 a.m. on October 1, 2012, Employee #1, a 69-year-old female teacher with the University of California, Santa Barbara was leaving the classroom when Employee #1's feet became entangled, tripped and caused her to fall forward onto desks. Employee #1 was transported to Cottage Hospital, hospitalized and treated for a broken right arm. The University of California, Santa Barbara is a public research university of the University of California system. Employee #1 had been employed with the University of California, Santa Barbara for approximately twelve years. Employee #1 was exiting the classroom at the time of the incident. Interviews were conducted with Employee #1 and an onsite coworker.
