Investigation Summary
At approximately 9:30 a.m. on October 1, 2012, Employee #1, a 52-year-old male truck driver with Main Electric Supply Company, had just delivered an order to an electrical contractor working in West Hills, as he was putting the gate back on the truck bed, his wedding ring caught on a metal part of the gate amputating the tip of his left ring finger. The electrical contractor walked with Employee #1 across the street to the hospital with the tip of the finger. X-rays were taken and Employee #1 advised to see a hand specialist. Employee #1 underwent surgery on October 2, 2012 which entailed shaving the bone and a skin graft. Interviews were conducted of the coworkers and supervisors. The employer, Main Electric Supply Company was a wholesale distributor for electrical contractors with approximately two hundred and twenty employees with seven locations.
