Investigation Summary

Investigation Nr: 201615374
Event: 11/07/2012
Employee Lacerates Hand in Rotating Saw Blade

On November 7, 2012, Employee #1 was stacking pallet stock. The production stopped and the coworkers were trying to figure out why it had stopped. The first step was to check the trim saw to make sure the vacuum pipes were not plugged. The trim saw was locked out prior to performing this task. The employee unhooked the vacuum hose and shook it while beating on the side of the hose just in case it was plugged. Employee #1 then went to check out the clean-out chute (located just below the saw) to see if it was plugged. The employee raised the trap door and put his right hand inside while the trim saw was still rotating. The saw caught his glove pulling his hand into the saw rotating blade. Employee #1 was transported to an area hospital, where he was treated for lacerated right thumb, ring and pinky fingers. The employee remains hospitalized.
