Investigation Summary

Investigation Nr: 202626610
Event: 05/15/2012
Employee Injures Finger in Feed Roll

On May 15, 2012, Employee #1 was operating an Oliver Manufacturing Vinyl Cutting Machine in order to process a customer's order for 34 ft. of Duality Premium Plus Vinyl Flooring. During the roll-out process the feed roll became low on material. The tightness of the wrap on the roll forced the material to jump on top of the two feed rollers. The employee attempted to correct the rollers by placing the feed roll of material back into the proper position and manually releasing the securing feed roller. Employee #1 was using the power of the machine, by jogging forward and backward, to force the feed roll back into its proper position. The employee had his left hand on the edge of the feed roll of material while this operation was being performed; his left index finger got caught in the in-running nip points of the feed rollers. While Employee #1 was trying to free his finger the roller tore skin and muscle tissue from his left index finger. Employee #1 reported to an area clinic, where he was treated and released.
