Employee Is Caught in Mast of Forklift and Dies

On July 2, 2012, Employee #1 was operating a Caterpillar (Model Number EC25K) ride-on forklift to load palletized bales of cardboard in a semi-trailer for recycling. During the activity, he raised himself up out of the operator's seat and reached through the vertical space between the top of the mast and the front edge of the roof of the operators cab. Employee #1 inadvertently got squeezed by the mast of the forklift impinging upon his torso. This space was 8 in. tall, and 10 in. away from the mast at a maximum. He sustained crushing injuries to his torso and several internal injuries. Employee #1 was transported to a general hospital, where doctors decided to transport him to the shock trauma center of another hospital. At this medical center, he died later on that same day.
