Investigation Summary
Employee #1 was working alone operating a full revolution mechanical power press. Employee #1 was making copper automotive gaskets, and the press was guarded by using pull backs. Employee #1 stated the pull backs were being used when the accident took place. Employee #1 accidently got his hand caught working the press. Employee #1 lost his little finger, middle finger to just a stub,ring finger and his index finger to the first knuckle. Employee #1 was taken to a local hospital for surgery.
