Investigation Summary
At 1:30 a.m. on June 28, 2012, Employee #1, a machine operator, started work at 10:00 p.m. and went directly to fabric machine Number 1405. He conducted the set-up on the machine and started making 8 foot by 12 foot wire (6 gauge) panels. Around 1:25 a.m., Employee #1 was walking from the back of the fabric mesh machine to the control center of the machine and realized something was wrong with the machine. Employee #1 ran to the emergency shut off to shut down the machine. Employee #1 noticed a small fire under the machine and told a coworker to get the supervisor. Employee #1 opened the guard gate and looked under the machine but could not see clearly what kind of fire it was. Employee #1 leaned over the machine by placing his left hand on the mesh wire on the table and his right hand was close to the lever. The employer did not ensure the power control was located to prevent accidental contact with parts of the body. Employee #1 inadvertently hit the lever, which released the pressure on the clutch of the table, causing the table to go back to the start position. Employee #1's fifth finger was amputated when it was caught between the mesh wire and the table.
