Investigation Summary
On February 18, 2012, Employee #1, a tool pusher, was trying to connect a hose underneath a chemical tote that was elevated twelve feet in order to gravity feed a chemical treatment into the heater treater. A forklift operator extended the boom of the rough-terrain forklift out to its maximum length when the rear tires lifted off the ground causing the forks to tilt forward. This caused the large chemical tote to fall from the forklift, crushing Employee #1 against the existing piping.
