Investigation Summary

Investigation Nr: 202720421
Event: 03/10/2014
Mason Climbing Scaffold Falls and Fractures Ribs

At approximately 3:00 p.m. on March 10, 2014, Employee #1, a brickmason/stonemason employed by Cornerstone Masonry Services, Inc., was performing masonry work at a commercial building. He was assigned to install joist well plates using a metal scaffold. He was climbing the outside of the scaffold when he slipped and fell approximately 14 feet to the ground. Emergency services were called, and Employee #1 was transported to the hospital, where he was admitted and treated for multiple rib fractures. This event was reported to Cal/OSHA by the employer at 4:05 p.m. on March 10, 2014. The subsequent investigation determined that Employee #1's fall was due to the employer not ensuring that a ladder was provided to access all of the scaffold platform.
