Investigation Summary
At approximately 12:00 p.m. on July 15, 2013, Employee #1 needed to climb a tree. He used an extension ladder to gain access to the upper portion of the tree. As he tied the ladder to the tree, the ladder tilted and caused him to fall. Employee #1 grabbed a branch on the way down, but it broke, sending him down. He fell upon a hilly ground and rolled down to the roadway, sustaining unspecified injuries. Employee #1 was transported to a medical center, where he was treated and then hospitalized.
