Employee's Leg Is Crushed When Run over by Forklift

At approximately 2:43 p.m. on February 27, 2013, an employee was jump starting a roll-off trailer for a customer in the yard. A coworker was operating a Caterpillar forklift, rated capacity 5,500 lbs, Model Number PD10000, Serial Number T19D80038. There was a narrow space on the left side of the roll-off trailer, so the coworker could not enter the area straight. The coworker then backed up his fork lift at which time the fork lift ran over the right leg and foot of the employee, causing serious crushing injuries. The employee was hospitalized and treated for a fractured leg.
