Employees Gets Struck by Pre Tensioned Cable

At approximately 7:15 a.m., on November 12, 2012, Employee #1 from Oldcastle Precast Inc., suffered a fatal injury accident, and Employee #2 sustained a serious injury. A double 24" Octagon form, designated Bed #2 (consisted of A & B form sections) with east to west orientation, used to cast concrete piles. Each form section has 16 steel cables. Prior to the accident, the double form (A & B) was subjected to tensioning stress of 649,600 pounds by a hydraulically operated Stress machine. After pre-stressing was completed, Employee #1 and Employee #2 began working from the west end of the form "A" when its' dead-end bulkhead section tore loose and both employees were struck. Employee #1 suffered fatal head trauma injuries and Employee #2 suffered multiple serious fractures to his left leg. Employee #1 was pronounced dead at site by the emergency personal. The cause of the accident was that the form "A" section of the double 24" octagonal form was overloaded by stressing the tension strands beyond the designed capacity of the form. The San Bernardino County Sheriff's Department Coroner was notified of the fatality. Employee #1 was transported by Deputy Coroner to its' facility. Paramedics transported Employee #2 to Arrowhead Regional Medical Center for treatment. Surgery was performed and Employee #2 was hospitalized for two weeks.
