Investigation Summary
At approximately 8:30 a.m.on October 06, 2012, Employee #1's main duties were to help remove the I Beam from the transformer. There were two forklifts that were used to set the I Beam on the donnage. EE#1 instructed by a coworker to go stand in an area to watch as a spotter to ensure that the I Beam did not puncture the air hoses. As Employee #1 was standing in an area close to the operation, the two forklifts were attempting to lower the I Beam and with a crib block under the center of the beam, this caused the I beam to begin to tip. The I Beam tipped over and struck Employee #1 causing multiple fractures. Employee #1 was hospitalized for three weeks with miltiple injuries.
