Investigation Summary
At approximately 1:10 p.m. on July 16, 2102, Employee #1 was walking out of the Emergency Room (ER) double doors, when she tripped and fell, face first, over a rolled up rug lying on the sidewalk just outside and between the entrances to the ER and Admitting Main Lobby. A visitor of the hospital witnessed the fall and reported it to hospital personnel. The rolled up rug was seen by Employee #2 earlier that day, propped vertically against the stoned wall around the corner of the ER entrance. It is unknown how the rug fell to the ground and how long the rug was on the ground prior to the incident. The rug involved in the incident has been removed from the hospital by the vendor as they are cycled in and out from the hospital.
