Investigation Summary
At approximately 6:30 p.m. on July 3, 2012, Employee #1 was working at a facility that converts and finishes tissue papers. He was involved with an activity to remove an empty paper roll. To release the paper roll, its holding device was slid out. The chock fell to the ground and struck Employee #1's right foot. He sustained crushing injury to this foot, including an amputation of its fourth digit. Employee #1 was transported to a medical center, where he underwent surgical procedures and was then hospitalized.
