Investigation Summary
At approximately 8:20 a.m. on March 29, 2012, Employee #1 inserted a piece of cardboard under a lockable caster wheel of a moveable belt conveyor. While getting up, he placed his right hand on the guard cover of the chain and sprocket chain drive of the conveyor for support. The cover guard had an opening of 4 inch by 2 inch at its rear with nip points, located at 21.5 inches above the floor level. Employee #1's right hand got caught between the moving chain & sprocket chain drive of the conveyor causing his right hand little finger to be amputated and lacerated the middle and ring fingers. He was taken to Riverside Community Hospital, Corona, CA, where surgery was performed the same day and he was hospitalized for less than 24 hours.
