Investigation Summary
On January 29, 2012 Employee #1 went to check a silo and the dirty flake pipes (storing ground-up plastic drink bottles that run from the grinder to the storage silo) due to line being clogged. Employee #1 accessed the roof via a JLG lift and was on the roof without using a safety harness and lanyard (approx. 20 feet above ground). Employee #1 used a 24 ft. extension ladder (brought up with him via the JLG lift) extended out approximately 15 to 18 ft. from the roof to get to the silo and pipes. These were 10 to 12 ft. above the roof and approximately 3 to 5 ft. away from the edge of the roof. The upper end of the ladder was resting on the pipes/pipe brace. Employee #2 also accessed the roof without a safety harness and lanyard earlier with Employee #1 to perform this exact same job. At that time, Employee #2 held the ladder for Employee #1. At the time of the accident, Employee #2 was on the ground below. The ladder was placed on unsecure footing (corrugated roof with pieces of plastic bottles laying on the roof). When Employee #1 was about 3/4 of the way up the ladder, the ladder slipped and slid back. Employee #1 attempted to grab onto one of the dirty flake pipes. Employee #1 fell approximately 30 feet to the ground below receiving multiple injuries to the body and head. Employee #1 was taken to the hospital where he was placed on life support and died a week later on February 5, 2012 from these injuries.
