Employee's Finger Amputated in Cut of Saw

At approximately 8:30 a.m. on June 30, 2011, Employee #1 was working on a cut off machine. He was making fittings out of a 1.25-in. pipe and the end of the pipe had to be cut off. The pipe was fed through the machine and it came out too far. With his left hand he pushed the feed handle to the left instead of the right which did not shut off the feeder. He reached to push the pipe back with his hand instead of using the pliers and pinched his right middle finger between the pipe and the stopper. Employee #1's finger was amputated and he was hospitalized.
