Investigation Summary
At approximately 2:00 p.m. on November 2, 2011, Employee #1 was working as a laborer for DVM Builders, LLC, in Yerington, NV. He was working on a corrugated metal roof with pitches of approximately 5-in-12 and 2-in-12. He was installing corrugated metal covers on skylights that had missing or damaged plastic covers. Employee #1 had partially installed corrugated metal on a skylight located near the peak of the roof. Employee #1 then walked down the roof. In the process, he fell through a skylight. Apparently, he had been carrying a piece of the old plastic cover from the skylight he had been working on. That piece of plastic was found later by OSHA compliance and safety officers below the western eave. A bigger piece of plastic was found near the skylight Employee #1 fell through. Employee #1 fell approximately 18 feet, 11.5 inches (5.78 meters) to the concrete floor inside the building. Employee #1 may have fallen while moving old pieces of plastic skylight covers. Employee #1 sustained injuries to his wrist, face, and head, including bleeding of the brain stem. Employee #1 was transported by air ambulance to Renown Medical Hospital in Reno, NV. He was hospitalized.
