Investigation Summary
At 10:30 a.m. on November 26, 2011, Employee #1, a 35-year-old noncontract field worker employed by JAL Berry Farms, was walking from a field. Employee #1 was struck by an industrial flatbed truck, and she sustained a fractured hip. She was taken to San Jose Regional Medical Center and was hospitalized for more than 24 hours. The Worker's Compensation Manager reported the accident at 4:52 p.m. on November 26, 2011.
