Employee on Atv Hits a Tree and Dies at the Scene

At approximately 7:50 p.m., on September 20, 2011 Employee #1 was working on a crew responsible for the operation of the main pump of a dairy farm. Employee #1 was on an all terrain vehicle (ATV) and had just delivered a gasket to repair a leak in one of the above-ground hose couplings out in the field. As he was returning to the main pump area, he went off the dirt road into the ditch where the ATV's front right tire struck a tree and ejected him. Employee #1 was thrown from the vehicle, struck a tree, and died as a result of his injuries. Employee #1 had not received training on the safe operation of the ATV.
