Investigation Summary
On August 25, 2011, Employee #1, a press assistant with Vertis Communications, was operating a Heidelberg Harris stacking machine (Model Number RS-3010S). Finished bundles would be removed from the stacker discharge table and palletized. Bundles would be bound with plastic strapping, depending upon customer requirements. The machine discharged the bundles by belt 6 to 8 in. out onto an air table for ease of handling. There was no guard on the discharge chute to prevent an operator from reaching into it. When Employee #1 noticed there were two bundles in the stacker discharge chute, he reached over the first one to pull out the second one, which was closest to the stacker operating mechanism. His finger was partially amputated by the operating mechanism.
